Registering for Online Services

Ask at reception for online access to your medical record.  You can also print off the form(s) and return to us.  You can also email the completed form(s) to [email protected]

Online Access to services for another patient is for dependent children under 16 years old, someone who lacks competency in managing their own healthcare or a patient who wishes to give you access to manage their online services.

You must be registered yourself before you can apply to look after someone else’s record.

Over 16’s can also register to manage their own online services if we feel they are able to understand and agree to the terms of the agreement in the application form.

Child or Dependent Online Access to Services for Another Patient 2017

Using the online services, you can:

  • Amend your contact details
  • Book an appointment – we have appointment available for GP, Blood Test, Blood Pressure, Smears, Long Term Condition, Contraception Reviews
  • Cancel an appointment –
  • Check appointment details
  • Request a repeat prescription
  • Request to view your full medical record.  Please complete the Access to Full Medical Record Agreement

Once you have access to your record, you can then request to view your entire computerised record, but we need you to fill in the Access to Full Medical Record Agreement and return to the secretaries at the surgery.

Access is always at the discretion of your doctor and can be withdrawn at any time.

You will need a user name and a password to the access the system. Our computer system will generate your password and you must change this the first time you use this service.

Usage Policy

Please use this service responsibly. In the case of any abuse of the service, your GP practice can revoke your log-in details, stopping you accessing the service. Examples of irresponsible use of the system include, but are not limited to:

– Booking appointments you have no intention of attending
– Repeatedly booking and then cancelling appointments
– Repeatedly requesting prescriptions that you do not need

Site Privacy Policy

TPP* is committed to protecting your privacy online. The personal information you enter on this website is strictly controlled. Information entered is available only to members of staff with appropriate access rights at our GP practice – ie. those managing appointment booking, repeat prescribing and patient registration. Your personal information will not be shared with any third parties. Your personal information will not be sold to any third parties.
