How to register

We welcome registration from patients living in, or moving to, the practice area.   Please click here to see our boundary.

Register at the surgery

You need to complete a registration (GMS1) form and health questionnaire (New Patient Check Form) which is available at the Reception desk.

To save time you can also download and print this registration form. This is known as a GMS1 Form.  Please complete the forms and bring to reception.

For adults and children aged 6 years and older, please complete this form:-  New Patient Check Form Combined 2022 and bring to your new patient check appointment.

For children and babies aged 5 and under, please complete this form:-New Patient Check Form Child 2022

Once all forms have been completed you would need to book an appointment with one our Practice Nurses for a New Patient Check appointment. Please call 01924 384498 option 1 or call into reception for our staff to book this for you.

We will only offer one appointment to attended, if you do not attend your appointment or fail to notify us we will not be able to go ahead with registration. You will need to register at another practice.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

If you require a interpreter for the appointment please can you let reception know so we can make the appointment time slightly longer.
