Register at the surgery

You can complete the registration form above and a health questionnaire (New Patient Check Form) at the Reception desk.

Once all forms have been completed you would need to book an appointment with one our Practice Nurses for a New Patient Check appointment. Please call 01924 384498 option 1 or call into reception for our staff to book this for you.

We will only offer one appointment to attended, if you do not attend your appointment or fail to notify us we will not be able to go ahead with registration. You will need to register at another practice.

You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

Eligibility

Everyone in the UK has the right to free care from a GP. If you don’t have proof of ID or address,
you can still register. Immigration status or nationality don’t matter, our reception won’t ask for immigration
documents and won’t share your information with the Home Office unless serious crime is involved.

We welcome registration from patients living in, or moving to, the practice area.   Please click here to see our boundary.

If you require a interpreter for the appointment please can you let reception know so we can make the appointment time slightly longer.
