Non-NHS services

The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services are displayed in the surgery or below:

Homestead Medical Centre – Private & Professional Fees May 2019

  • Short Letter / to Whom it may concern / Signature on forms – £25.00 (Information that can be taken directly from medical records is NOT chargeable and patients are requested to register for online access for such information)
  • Private Prescription £20.00
  • Private Sicknote £20.00
  • Duplicate Sicknote £10.00
  • Driving Licence Form countersignature £30.00
  • Taxi/HGV/PSV/LGV Medical – Examination and Report – £130.00
  • Taxi Concession over 65 years only – £95.00
  • Taxi Medicals for Non Patients – £150.00
  • Request for a GP Report of any kind – £150.00
  • Private Consultation – 30 minutes – £200.00
  • Request for completion of a form where a clinical opinion is required – £50.00
  • Army Questionnaire – set by MOD, currently £65.00
  • Fitness to Travel – £40.00
  • Power of Attorney – patient needs to be seen – £200.00
  • Private Adoption/Fostering Medical – (WMDC set own fees – currently £74) – £120.00
  • DNA swabs – £30.00
  • Employer request for Medical Report – £150.00
  • Health Certificate for University/College/Teacher & Nurse Training – £20.00
  • Character Reference – (if known patient personally for 3 years). – £50.00
  • Influenza for travel – £30.00
  • Vaccination Certificates – £25.00
  • Subject Access Requests (SAR) requests for information from Medical Records Free via online access
  • Hep B (x3 vaccinations) for occupational reasons NOT travel £50.00 for 3 doses
  • Rabies/Malaria/Meningitis ACWY – Private Prescription £30.00

A 50% non-refundable deposit must be paid upon request.

The Doctors will not complete if no evidence of a deposit
