Information Regarding Private Work
Private paperwork, for example, holiday, employment or insurance reports is classified as private work. GPs are not obliged to complete these forms as their primary responsibility is NHS work.
Due to the prioritisation of NHS duties, the completion of private paperwork may take 3 – 4 weeks or longer during busy periods.
Important Information:
- Fees for private paperwork are determined by the practice.
- Payment is required in advance.
- The GP reserves the right to decline completion of the form if the fee is not paid.
- GPs may also decline to complete the form if they feel unable to provide the information requested.
- Even if a form only requires a GPs signature, it is a professional responsibility that must be undertaken with care. By signing, the GP is certifying the accuracy of the information in line with their professional responsibilities under the General Medical Council (GMC) guidelines. This means the GP must review the content carefully to ensure it is correct, as any errors could put their professional registration at risk.

- Short Letter / to Whom it may concern / Signature on forms – £25.00 (Information that can be taken directly from medical records is NOT chargeable and patients are requested to register for online access for such information)
- Duplicate Sicknote £10.00
- Taxi/HGV/PSV/LGV Medical – Examination and Report – £130.00
- Taxi Concession over 65 years only – £95.00
- Taxi Medicals for Non Patients – £150.00
- Request for a GP Report of any kind – £150.00
- Private Adoption/Fostering Medical – (WMDC set own fees – currently £74) – £120.00
- Employer request for Medical Report – £150.00
- Health Certificate for University/College/Teacher & Nurse Training – £20.00
- Access to Medical records – patients should be asked to sign up for systmOne online or the NHS App
- Photocopying – 45p per copy
- Vaccination Certificates – £25.00
- Hep B (x3 vaccinations) for occupational reasons NOT travel £50.00 for 3 doses
- Rabies/Malaria/Meningitis ACWY – Private Prescription £30.00
- DNA swabs – we no longer do private DNA swabs they need to come from cellmark and they pay for them no the patient – £150 for information only
- Army Questionnaire – these are paid by the Army themselves and not the patient – £150 for information only charge to the Army
Full payment must be taken in advance from the patient before completing any forms.
