The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services are displayed in the surgery or below:

- Fitness to Travel – £40.00
- Power of Attorney – patient needs to be seen – £200.00
- Private Adoption/Fostering Medical – (WMDC set own fees – currently £74) – £120.00
- DNA swabs – £30.00
- Employer request for Medical Report – £150.00
- Health Certificate for University/College/Teacher & Nurse Training – £20.00
- Character Reference – (if known patient personally for 3 years). – £50.00
- Influenza for travel – £30.00
- Vaccination Certificates – £25.00
- Subject Access Requests (SAR) requests for information from Medical Records Free via online access
- Hep B (x3 vaccinations) for occupational reasons NOT travel £50.00 for 3 doses
- Rabies/Malaria/Meningitis ACWY – Private Prescription £30.00
A 50% non-refundable deposit must be paid upon request.
The Doctors will not complete if no evidence of a deposit
